I am an out-of-network provider and do not accept insurance. Payment is due at the time of service. Please contact me directly for my current rates and fees.

For possible out-of-network reimbursement*, I will send you a monthly “Superbill” and Statement for you to submit to your insurance company. *Please note that reimbursement is not always guaranteed. Please check with your insurance provider for out-of-network benefits. I use the following CPT codes most often:

• Individual Psychotherapy, 60 Minutes (CPT Code: 90837)

• Individual Psychotherapy, 45 Minutes (CPT Code: 90834)

As a provider, I have chosen to separate (“opt-out”) from Medicare. Because I opted out, Medicare requires that I have you sign a private-pay medical services contract before starting treatment services if you are a Medicare beneficiary.

I am a contracted provider with Lyra Health EAP (Employee Assistance Program). Please check with your employer if you have Lyra Health EAP benefits.
